Brian’s SSA (case manager) Will makes sure that he talks with Brian about his needs and then helps him put services in place to meet those needs. He has had a large impact on helping Brian transition from a waiver home out of county to living locally. He also assisted with Brian moving into a home owned by the county board/Residential Administrators when he returned home and we were at risk of losing our housing.”
